Job Summary:
We are seeking a compassionate and dedicated Disability Home Care Social Worker to join our team. In this role, you will be responsible for providing essential support and assistance to individuals with disabilities in a home care setting. Your primary goal will be to enhance their quality of life and promote their independence through personalized care and social services.

Responsibilities:
1. Conduct comprehensive assessments of individuals with disabilities to identify their unique needs and preferences.
2. Develop individualized care plans in collaboration with clients, their families, and interdisciplinary teams.
3. Provide counseling and emotional support to clients and their families, addressing issues such as adjustment to disability, coping mechanisms, and advocacy.
4. Coordinate and facilitate access to community resources and services, including medical care, therapy, vocational training, and recreational activities.
5. Advocate for clients’ rights and entitlements, ensuring they receive appropriate benefits and services.
6. Monitor clients’ progress and reassess needs regularly, adjusting care plans as necessary.
7. Document all client interactions, assessments, and interventions accurately and timely.

Requirements:
1. Bachelor’s or Master’s degree in Social Work or a related field.
2. State licensure or certification as a Social Worker (LCSW or equivalent).
3. Proven experience working with individuals with disabilities, preferably in a home care or community-based setting.
4. Strong knowledge of disability rights, legislation, and available support services.
5. Excellent communication and interpersonal skills, with the ability to establish rapport with clients from diverse backgrounds.
6. Ability to work independently and as part of a multidisciplinary team.
7. Empathy, patience, and a genuine desire to improve the lives of individuals with disabilities.
