Job Title: Registered Nurse

Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our disability home care team. In this role, you will play a crucial part in delivering high-quality healthcare services to individuals with disabilities in their home environment. You will collaborate with interdisciplinary teams to assess, plan, and implement care strategies tailored to meet the unique needs of each client.
Responsibilities:
1. Conduct comprehensive health assessments and develop individualized care plans for clients with disabilities.
2. Administer medications, treatments, and procedures as prescribed, ensuring adherence to safety protocols and regulations.
3. Monitor clients’ vital signs, symptoms, and overall health status, and promptly address any changes or concerns.
4. Provide hands-on assistance with activities of daily living, such as bathing, dressing, grooming, and mobility support.
5. Educate clients and their caregivers on health management techniques, medication administration, and preventive care measures.
6. Collaborate with physicians, therapists, social workers, and other healthcare professionals to coordinate holistic care for clients.
7. Document all assessments, interventions, and communications accurately and maintain up-to-date client records.
Requirements:
1. Valid RN license in the state of practice.
2. Minimum of 5 years of clinical experience, preferably in-home care or disability services.
3. Proficiency in performing nursing assessments and implementing care plans.
4. Strong knowledge of disability-related health conditions, medications, and treatments.
5. Excellent communication and interpersonal skills, with the ability to establish rapport with clients and their families.
6. Ability to work independently and make sound clinical judgments.
7. Flexibility to adapt to changing client needs and schedules.
8. CPR certification and basic life support (BLS) training.
