Positons we recruit for:
Direct Support Professional (DSP)
Provides hands-on, person-centered support to individuals in their homes and communities, including daily living skills, community participation, personal supports, documentation, and implementation of individualized goals.
Lead Direct Support Professional (Lead DSP)
An experienced DSP who provides shift leadership, supports staff orientation, assists with documentation and daily operations, and helps ensure consistent implementation of individual support plans.
House Manager / Residential Manager
Oversees the day-to-day operation of a residential home, including staffing, household routines, documentation, appointments, individual needs, safety, and coordination of person-centered supports.
Program Coordinator
Coordinates individual services, schedules, documentation, PCP/SIP implementation, appointments, activities, and communication among individuals, families, staff, and members of the support team.
Residential Program Coordinator
Provides coordination and oversight for residential services, including Community Living, Supported Living, Shared Living, and other DDA residential supports.
Program Manager / Residential Program Manager
Provides operational oversight for Gracewell programs and residential homes, supervises House Managers and DSPs, monitors service quality, and supports compliance with DDA and agency requirements.
Community Support Specialist
Provides community-based support focused on independence, skill development, socialization, recreation, personal interests, and meaningful participation in community life.
Personal Support Staff —
Provides individualized Personal Supports in the individual's home and community while promoting independence, choice, safety, and progress toward person-centered goals..
Share Living Provider
Provides person-centered support through Gracewell's Shared Living service model for individuals whose approved services and preferences are appropriate for this type of arrangement.
Respite Support Staff
Provides temporary support, supervision, and assistance to individuals when respite services are authorized, helping maintain safety, routine, and continuity of care.
Certified Medication Technician (CMT)
Supports medication administration and documentation within the employee's authorized role and in accordance with applicable requirements, nursing delegation, and Gracewell policies.
Behavioral Support Staff / Behavioral Support Specialist
Supports medication administration and documentation within the employee's authorized role and in accordance with applicable requirements, nursing delegation, and Gracewell policies.
Behavioral Support Staff / Behavioral Support Specialist
Supports individuals with behavioral needs and assists with implementation of approved behavioral strategies, positive supports, de-escalation approaches, and individualized behavior plans.
Quality Assurance / Compliance Coordinator
Reviews individual records, staff files, service documentation, incident documentation, residential sites, and program operations to support quality improvement and DDA compliance
Training & Staff Development Coordinator
Coordinates employee orientation, required DSP training, competency tracking, CPR/First Aid, CMT-related training requirements, and ongoing staff development.
Scheduling / Staffing Coordinator
Coordinates DSP schedules and staffing coverage, manages call-outs and replacements, and helps maintain appropriate staffing across Gracewell programs and residential settings.
Administrative / Executive Assistant
Provides administrative support for office and program operations, including records, scheduling, correspondence, document organization, and coordination of routine business activities.
